David R. Hager, PharmD, FASHP

David R. Hager, PharmD, FASHP, is the Interim Chief Pharmacy Officer at Dartmouth Health and a Senior Director at Visante, the clinician-composed improvement company focused on transforming healthcare through pharmacy. Hager has served with Dartmouth Health for the last two years in a variety of leadership and consultant roles.

Prior to joining Dartmouth Health and Visante, Hager was the Director of Clinical Pharmacy Services at UW Health where he had accountability for: advancing inpatient pharmacist clinical practice; clinical productivity; inpatient drug spend reduction; relationships with school of pharmacy partners; staff competency and engagement; the transplant service line; and transitions of care. He was named PSW Pharmacist of the Year in 2016, Pharmacy Alumni Association Young Alumnus of the Year in 2019, Pharmacy Times Health System Next Generation Pharmacist of the Year in 2020, Fellow of the American Society of Health System Pharmacists in 2023, and PSW Good Governance Award winner in 2024.

Hager received his Doctor of Pharmacy degree from the University of Wisconsin-Madison School of Pharmacy. He completed a PGY1 Pharmacy residency at UW Health. He is a Clinical Associate Professor at the University of Wisconsin-Madison School of Pharmacy. Hager has multiple peer-reviewed publications in leadership, transplantation, transitions of care, and pharmacy practice and has authored five book chapters.
